A Learjet 35A (N135SH) asked to be topped off with Jet A + FSII/PRIST.
Instead, it received 674 gallons of fuel contaminated with Diesel Exhaust Fluid (DEF) — because a fueler accidentally poured the clear DEF into the wrong tank on the truck. Result?
• Erratic engine temperatures & fuel flow
• Right engine → total power loss
• Left engine → partial failure
• Yawing and “pulsing” mid-flight
